1.Athlete Information
1.Liability Waiver & Assumption of Risk
I, the undersigned parent/guardian, acknowledge that participation in basketball training, practices, games, tournaments, and related activities organized by Antz Hoopz involves inherent risks of physical injury. I voluntarily allow my child to participate and assume all risks associated with such participation. I hereby release, waive, discharge, and covenant not to sue Antz Hoopz, its coaches, trainers, staff, volunteers, agents, and affiliates from any and all liability, claims, or demands arising from participation in any Antz Hoopz activity, whether caused by negligence or otherwise.
2.Medical Authorization
In the event of an injury or medical emergency during any Antz Hoopz activity, I authorize the coaches and staff to seek emergency medical treatment on behalf of my child if I cannot be immediately reached. I agree to be responsible for all medical costs incurred.
3.Photo & Video Release
I grant Antz Hoopz permission to use photographs, video footage, or other media of my child taken during training sessions, games, or events for promotional, educational, or social media purposes without compensation.
4.Athlete Expectations
- I will show respect to coaches, teammates, opponents, and officials at all times.
- I will arrive on time and prepared for all practices, games, and team events.
- I will give my maximum effort in every drill, practice, and game.
- I will maintain a positive attitude and support my teammates.
- I will take responsibility for my actions on and off the court.
- I will prioritize academics and maintain passing grades.
- I will represent Antz Hoopz with class and integrity at all times.
5.Academic Commitment
- I will complete all homework and school assignments on time.
- I will maintain passing grades in all classes.
- I will communicate with coaches about any academic struggles.
- I will prioritize schoolwork over basketball when necessary.
- I will conduct myself as a student-athlete with pride and responsibility.
Failure to maintain passing grades or poor academic behavior may result in limited participation until improvement is shown.
6.Parent/Guardian Expectations
- I will support my child's commitment to the team and attend games and events when possible.
- I will ensure my child arrives on time and is picked up promptly.
- I will communicate respectfully with coaches and staff.
- I will support the coaching philosophy and avoid sideline coaching during games.
- I will model good sportsmanship and respect for officials, opponents, and other parents.
- I will fulfill all financial obligations on time as outlined in the payment policy.
7.Attendance & Payment Policy
- All payments are non-refundable. Sessions must be used within the applicable use window.
- A 24-hour cancellation notice is required to reschedule any session.
- Monthly membership sessions do not roll over to the next billing cycle.
- Cancel anytime before your next billing date. No refunds on completed billing cycles.
8.Agreement
By signing below, I acknowledge that I have read, understood, and agree to all terms, expectations, and policies outlined in this waiver. I certify that the information provided is accurate and complete. I understand that this agreement is binding for the duration of my child's participation in the Antz Hoopz AAU Basketball program.
